How to Submit Your Application

  1. Download the application above and complete every section. Type “N/A” where something doesn’t apply.
  2. Sign and date section 8, then save the completed application.
  3. Email it to ric.ralston@mic-lock.com.

Submitting this application does not make you a Mic-Lock affiliate. Mic-Lock will confirm approval and send program terms in writing.
